We are very lucky at Alexander Heights Family Practice to have lots of available support from Allied Health Professionals. This includes Pathology, Dietetics, Psychology, and Physiotherapy. These services can all be accessed at low or no cost for health care card holders and financially stressed people, when referred through a Medicare Chronic Disease Management Plan. For those not covered by concessions, a chronic disease management plan can lower the cost of visits.

We can organise a chronic disease management plan for any condition ongoing or likely to persist for longer than six months. Please ask for a management plan when booking.
