Management Plans

If you have a chonic condition, a chronic disease management plan is available through your GP. At our practice, we encourage patient participation in chronic disease managment. The requirements are that you have a chronic condition that has lasted or will last at least six months, and would benefit from pre-planned care.

The plan sets out the care for your medical issue over the next 12-24 months, and includes lifestyle targets as well as medical monitoring. Allied health and other health professionals can also get invovled, with up to five visits per calendar year total for a broad range of allied health workers. Examples of conditions covered would be diabetes, heart disease, asthma, COPD, chronic back or shoulder pain, or obesity with complications. 

Talk to your GP about organising a plan, or you can book an appointment with the nurse and the doctor if you would like to proceed.
